Workflow/Flowchart: Emergency Shelter (Congregate)
This article contains the 1) official workflow and 2) flowchart of data collection and data entry for congregate Emergency Shelter (ES) programs for DE-CMIS.
Please note that this is a general workflow and flowchart; some programs may have slight alterations to the content provided here.
EMERGENCY SHELTER WORKFLOW
For Large and Congregate Shelter Programs
- INTAKE
 
- Meet with client and complete all necessary intake paperwork.
 
CMIS DATA ENTRY
- BACK DATE MODE
 
- Set Back Date Mode to date and time of when the client checked into the shelter program.
- CoC guidelines must be adhered to, where data for shelter stays must be entered within three (3) days of client completing intake.
- CLIENTS MODULE - SEARCH
 
- Clients who have contacted CI or received services in the past will have a profile in CMIS. Some clients will need to have a profile created.
- CLIENT PROFILE
 
- Review client's Universal Data Elements (UDEs) and enter/update, if needed.
- HOUSEHOLDS
 
- Confirm that all household members being checked into shelter program are linked in a household. If not, add additional household members.
 
- Newly added household members will need to have their UDEs entered; Name, Social Security Number, and Veteran Status can be added in this tab.
- Add two Releases of Information (ROIs) for all household members under: 1) the Parent Provider and 2) the specific ES project the client is being checked into.
- ENTRY/EXIT – PROJECT ENTRY
 
- Click “Add Entry/Exit”.
- Select all household members that are being checked into shelter.
- Add an Entry/Exit under the specific shelter that the client is staying at.
- Review/update all questions on Entry Assessment, to accurately reflect client's situation as of their date of intake.
- DURATION OF SHELTER STAY
 
- Case Management tab – add clients’ case manager
- Case Plans tab – upload files relating to client’s case management
- Interims – record changes in a client’s information throughout their shelter stay.
- PROJECT EXIT
 
- Clients are to be exited from the project when they check out or do not stay at the shelter for a night.
- Select all household members exiting the project together.
- Select the Reason for Leaving and Destination that best reflects the client’s situation.
- Update the Exit Assessment with any information not collected at Entry or through an Interim.
