Workflow/Flowchart: Joint Transitional Housing/Rapid Re-Housing
This article contains the 1) official workflow and 2) flowchart of data collection and data entry for Joint Transitional Housing (TH)/Rapid Re-housing (RRH) programs for DE-CMIS.
Please note that this is a general workflow and flowchart; some programs may have slight alterations to the content provided here.
JOINT TRANSITIONAL HOUSING/RAPID RE-HOUSING WORKFLOW
- REFERRAL
 
- Receive referral from Centralized Intake (CI) through CMIS and email.
- Contact client.
- Explain program policies and procedures.
- Schedule client's intake and move-in.
- INTAKE
 
- Meet with client and complete all necessary intake paperwork.
 
CMIS DATA ENTRY
- CLIENTS MODULE/CLIENT SUMMARY
 
- Clients referred by CI will already have a profile in CMIS.
- Accept RRH Referral from “Outstanding Incoming Referrals” on the Client Summary tab.
- CLIENT PROFILE
 
- Review client's Universal Data Elements (UDEs) and update, if needed.
- HOUSEHOLDS
 
- Check that all household members being enrolled into program are linked under a household. If not, add additional household members now.
- SHELTERS MODULE
 
- Select program as Provider and click “View All” on the Shelters Module Dashboard.
- Un-hold bed(s) clients will move into.
- Begin Check-In process – Use the check-in button next to Head of Household in the Outstanding Referrals section ( ). ).
- CHECK IN – TH PROJECT ENTRY
 
- “Date In” should reflect date and time of client move-in.
- Assign Unit.
- (for families) Select all household members also moving in and Assign Unit.
- Add three Releases of Information (ROIs) for all household members under: 1) the Parent Provider, 2) the specific TH project the client is being checked into, and 3) the specific RRH project the client is being served by.
- Review/Update the Entry Assessment, as needed.
- All assessment questions (living situation, income, non-cash benefits, insurance, disability) should reflect the client's situation as of their date of check-in.
- ENTRY/EXIT – RRH PROJECT ENTRY
 
- Click “Add Entry/Exit”.
- Select all household members that are being enrolled.
- Add an Entry/Exit under the specific RRH project (“Provider”) that the client is being served by.
 
- All assessment questions from the TH check-in will transfer to the RRH entry, as long as all ROIs were entered.
- DURATION OF PROGRAM STAY
 
- Case Management tab – add clients’ case manager.
- Case Plans tab – upload files relating to client’s case management.
- Interims – record changes in a client’s information throughout their stay.
- Housing Move-In Date (HMID) – Through an interim, add HMID for all household members once they move into permanent housing.
- For a Joint TH/RRH program, the HMID will be when the client exits the TH program (skip to 8c).
- Annual Assessment – Needs to be added annually through an interim for clients enrolled in program for 365 days or more.
- Clients can be enrolled under multiple RRH Providers if they are receiving funding from them.
- If one RRH program is CoC-funded, the Entry and Exit Dates will have to encompass the entire time range that the client was served.
- If the later RRH enrollment is not CoC-funded, a second HMID will have to be entered to match the date of the new enrollment's Start Date.
- PROJECT EXIT
 
- Clients are to be exited from the RRH project when they stop receiving RRH assistance.
- If the client exits the program without being housed, the TH Exit Date will match the RRH Exit Date.
- TH – SHELTERS MODULE CHECK OUT
 
- Use check-out button next to Head of Household ( ) when exiting client(s) from TH program. ) when exiting client(s) from TH program.
 
- Select all household members exiting the project.
- Select the “Reason for Leaving” and “Destination” that best reflects the client’s situation.
- Update the Exit Assessment with any information not collected at Entry or through an Interim.
 
- Hold the now-Empty bed(s) until they are available for new Referral from CI.
 
- RRH – CLIENTS MODULE EXIT
 
- Select all household members exiting the project.
 
- Select the "Reason for Leaving" and "Destination" that best reflects why the client is exiting the program and where they are exiting to.
 
- All Exit Assessment information will be transferred from TH Exit Assessment. Provide any additional updates that may be applicable to client upon exit.
