PATH: How to Add Referrals

INTRODUCTION. This guide outlines how CMIS end users for PATH funded projects can add PATH referrals. This will allow referrals to be counted and reflected on the PATH report necessary for SAMHSA submissions.

 

 

1.      Activate the Enter Data As (EDA) mode before adding any referrals.

 

1.      From the upper right corner of the screen, click on "Enter Data As".

 



 

2.     Select the specific PATH project you will be adding referrals for by clicking on the plus icon.

 

3.      After selecting a project, the name of the project should show next to "Enter Data As" in the upper right corner of the screen.


  

2.    Double-check that the client has already been enrolled in your PATH project and the PATH-specific fields in the entry assessment have been completed.

 

3.      On the Service Transactions tab, click on “Add Referrals”.


 

4.     Select all the applicable household members by clicking on the check box next to their name.

 

5.      Under the Needs Assignment, select a Need listed under the Service Code Quicklist then click on “Add Terms”.

6.     Under the Referral Provider Quicklist, click on the drop down and select the only option: “PATH Referral Provider (943)”. Then, click “Add Provider”.

 

NOTE: You may minimize the Search for Providers panel as this step is unnecessary, given that you select project 943.






 

7.      Fill out the Referral Data section, specifically the following fields, to ensure data will populate on the PATH report.

a.     Type of PATH Referral

b.     If any “Type of PATH Referral” made, select Outcome

NOTE: The Needs Referral Date will automatically set to the date on your computer settings. It is possible to change this to the past, to accurately reflect when the referral was made.

 

8.     In the Referrals Table, tick the box of the need/s that is/are being referred.




 

9.     Scroll down and click on “Save ALL”.

 

10.  Run a PATH report to double check if the referrals added are appearing on the Referrals Provided table.
